Case Manager Position

QUALICATIONS:

• Bachelor’s degree in human services field; or currently licensed Registered Nurse; Can substitute up to two years of full-time work experience involving direct contact with people in overcoming social, economic, psychological or health problems in a human service field for two years of educational requirement.
• Must be able to work well with others and possess good oral and written communication and organizational skills. Experience in working with the elderly and knowledge of community resources a strong plus.
• Must have reliable transportation, valid driver’s license, auto insurance and be able to travel in and outside our service area, as job requires.
• Must have a good working knowledge of computers and common programs, (i.e. Word, Outlook, Excel and Access).
• Successfully pass a Criminal background and Dependent Adult Abuse check before hire.

JOB SUMMARY:
• Case Managers work under the direction of the Case Management Coordinator.
• Directs the planning, implementation, and evaluation of Case Management program for assigned consumers within the parameters of the Case Management Assurances.
•Collaborates with Service Providers and the Department of Human Services, when needed, to coordinate consumer care.

RESPONSIBILITIES AND DUTIES:

• Responsible for developing a rapport with the consumer; arrange for, and administer, the screening tool and the Comprehensive assessment in a professional manner.
• Reviews, finalizes and implements the consumer service plans, while consulting with consumer, family, and providers of direct care in a professional and courteous manner.
• Maintains complete file of all consumer contacts, including telephone contacts and quarterly face-to-face visits.
• Evaluate changing consumer needs and service adequacy.
• Complete a state based CMPFE case manager training within six months of hire. Attend other training and meetings as required.
• Evaluates client needs and service adequacy on a quarterly basis.
• Enter and update all forms associated with the Seamless computer program developed by the Department of Elder Affairs.
• Enter and update records for the “Individualized Services Information System” or “ISIS” computer program developed by Iowa Department of Human Service.
• Ensure Consumer Notice of Decision is completed and sent to all assigned consumers and their providers of services.
• Provide a daily log of work completed for Consumers for Elderly Waiver Billing and Quality Assurance purposes.
• Maintains and submits statistical records to Case Management Coordinator in a timely manner.
• Keeps Coordinator informed of accomplishments and problems/issues within assigned county (ies).
• Follows and maintains program Assurances.
• Conducts orientation for new service providers on CMPFE process and paperwork when needed.
• Provides public education on the CMPFE program throughout the 8 county area.
• Provides support to other CMPFE case managers when needed.
• Perform other duties as deemed necessary for the success of the agency.
